Information And Referral

Staff link people in need with the appropriate community agency or service, as a result of a phone call, e-mail, or an in-person visit.

Clearinghouse

Members monitor the provision of emergency material assistance (rent, utilities, food, and clothing) to families and individuals in Lycoming County by using a computerized database. This eliminates unnecessary duplication of services and improves the coordination among the member agencies.

Telephone Reassurance

Volunteers provide daily or weekly phone calls or e-mail to individuals who are lonely and do not get out much or need additional contact.

Advance Care Plan

The North Central PA Advance Care Planning Taskforce helps individuals ensure that their values, wishes, and choices in health care will be respected. Linkage Lycoming coordinates meetings between the customer and facilitator to help individuals choose and document their end of life decisions.
