 ed after Aug. 2023)\nLegal (custody agreement\, PFA\, etc.)\nProof of Resi
 dency (utility bill preferred\, or lease\, rental agreement\, or property 
 taxes)\n \n*Northern Tioga School District Policy 201 states the Superinte
 ndent or designee shall require that the parent/guardian of each student w
 ho registers for entrance to the school shall exhibit his/her birth certif
 icate or similar documentation as submit proof of age and birth date.\n \n
 *Required immunizations:\n            - 4 doses diphtheria\, tetanus\, and
  acellular pertussis (DTP)  (1 dose on or after the fourth birthday)\n    
         - 4 doses polio (4th dose on or after 4th birthday and at least 6 
 months after previous dose given)\n            - 2 doses measles\, mumps\,
  rubella (MMR)\n            - 3 doses hepatitis B\n            - 2 doses o
 f varicella (chicken pox) or written statement by family physician of havi
 ng had the disease with the date or age of the child.
